Michelle K. Doroz, D.O.

Dr. Michelle K. Doroz is board certified in Family Medicine. She graduated from Texas College of Osteopathic Medicine in Ft. Worth, Texas and completed her family practice residency at Phoenix Baptist Hospital in Phoenix, AZ. While in medical school, she received the National Health Service Corps Scholarship and fulfilled her commitment at Winslow Indian Health Care Center in Winslow, AZ. She then went on to work in Grand Canyon National Park at the Grand Canyon Clinic. Her interests in Family Medicine include newborn care, pediatrics, preventative care, and geriatrics. Dr. Doroz has lived in Flagstaff since 2004 and enjoys all the outdoor activities it has to offer.


Keith_Arnold, M.D.

Dr. Keith Arnold

Dr. Keith Arnold is a board certified Family Medicine Physician. He recently completed his training at the Mountain Area Health Education Center in Asheville, North Carolina. He is a graduate of University of Arizona Medical School where his interest in community health, leadership and policy inspired him to complete the combined Masters of Public Health / M.D. program. As an undergraduate Keith attended U.C. Berkeley and studied International Political Economy. His passion for medicine arises from personal experiences caring for loved ones during his youth and integrates his desire to build meaningful relationships and uplift communities. Dr. Arnold treats patients across the lifespan from newborn to geriatrics. His areas of focus include: early childhood development, newborn, pediatrics, in-office procedures, preventative care, mental health and substance abuse treatment. He believes in partnering with patients to effectively manage chronic conditions and achieve mutual health goals. He looks forward to providing holistic, patient-centered care to the residents of Flagstaff and beyond. Se habla español.


Chad Taylor D.O.

Dr. Chad Taylor was born in Fargo North Dakota, but was raised in Eden Prairie, a little suburb outside of Minneapolis. He attended Minnesota State University in Mankato where he earned his undergraduate degree in Human Biology with Minors in Chemistry and Political Science. After his undergraduate education, Dr. Taylor attended medical school at A.T. Still University where he received his Doctorate in Osteopathy. During his Family Medicine residency in Kingman, AZ, Dr. Taylor was recruited to join a dermatology team with the professionals at Mohave Skin and Cancer Clinic where he practiced and received special training for 5 years. Dr. Taylor believes in a cohesive, patient-centered, multidisciplinary clinical approach to offer the best medical care possible to his patients. On his free time, he enjoys running, playing racquetball, playing guitar, reading, writing, and spending time with his family. Dr Taylor will be seeing dermatology patients at our Flagstaff 4th St office only Thursdays 8am-5pm.


Shari Pressley M.D.

Dr. Shari Pressley is an Arizona native. She was born and raised in Phoenix, and moved to Tucson for undergraduate and medical school at the University of Arizona. She moved to Phoenix again for residency at Banner University Medical Center. After residency, she stayed on with her clinic for a number of years seeing patients and educating residents and medical students. She is very excited to be on her next adventure in Flagstaff along with her husband and 2 children.

Cameron Allen, PA-C, is a Phoenix native who graduated from Northern Arizona University with a Masters in Physician Assistant Studies. During his undergraduate studies he studied health sciences at Arizona State University while working as a pharmacy technician. Cameron has a passion for preventative medicine, research and public health initiatives. He is excited to join our Flagstaff team and better the health and wellness of the community. In his free time, Cameron is an artist and avid musician with a love for electric guitar, bass, and synth.

Meghan O'Donnell, M.D.

Dr. Meghan O’Donnell graduated from the University of Colorado with a major in Kinesiology and Applied Physiology. She earned her Doctor of Medicine from Drexel University College of Medicine, Philadelphia, PA, and performed a family medicine residency at University of South Florida College of Medicine, Bayfront Medical Center in St. Petersburg, FL. Board-certified, Diplomate, American Board of Family Medicine, Dr. O’Donnell specializes in family-centered medical care including pediatrics, women’s health and chronic disease management for all. Dr. O’Donnell joined Dr. Andrew at Flagstaff Family Care Clinic in 2008.


Elysia Small, PA-C

Elysia Small, PA-C graduated from the University of Florida, Gainesville, FL in 2007 with a bachelor’s degree in Nutrition and a minor in Japanese Language and Literature. She has several years of experience as a Rehab Tech and CNA. She has volunteered in many capacities including as a nursing home activities volunteer and as a mentor through Big Brothers Big Sisters. She graduated in 2016 with a Master of Science in Physician Assistant Studies from A.T. Still University in Mesa, AZ and recently moved to Flagstaff, AZ to join the team at Flagstaff Family Care. She enjoys working with all ages including children.

During her free time, Elysia enjoys many outdoor activities including hiking, rock climbing, caving, and canyoneering. She also enjoys an occasional artistic endeavor and spending time with family and friends.


Karen Damgaard, PA-C

Karen Damgaard, PA-C is a Flagstaff native. Born and raised in Flagstaff, she has always returned because she considers Flagstaff home. Karen earned a Bachelor of Fine Arts degree in Graphic Design from the University of the Pacific in Stockton, California. She later went to earn a Bachelor of Science degree in Zoology with a minor in Pre-med studies and Chemistry at Northern Arizona University. Prior to earning her Masters degree in Physician Assistant Studies at Idaho State University in Pocatello, Idaho, Karen worked for several years as a medical assistant in several private practices settings as well as a community health care facility. As a PA, Karen has spent the last 10 years serving the medical needs of the uninsured, underinsured, and privately insured people of Northern Arizona by continuing her work in a community health care center. She is excited to join the team at Flagstaff Family Care and looks forward to the challenges that lie ahead. Karen enjoys the field of family medicine because it allows her to practice medicine in a wide variety of areas while interacting with patients of all ages. Her greatest reward from practicing medicine is learning from her patients and watching them achieve optimal health.

In her spare time, Karen enjoys spending time outdoors and training for half-marathons, marathons and century bike rides. She also enjoys traveling, reading a good book and relaxing with her family, friends and dog.

Dr. Mary Porvaznik is Board Certified in Family Medicine. She completed medical school at the Uniformed Services University of the Health Sciences in Bethesda, MD. She completed her Family Medicine Residency at the Medical University of South Carolina in Charleston, SC. She served in the US Public Health Service with the Indian Health Service for 24 years in various jobs and locations, including Tuba City, AZ and Shiprock, NM and a 4 year tour teaching back at her medical school in Bethesda, MD.
She moved to Flagstaff 2 years ago with her teenage daughters and husband to take advantage of the educational resources and opportunities the city has available. Dr. Porvaznik cares for people of all ages and all walks of life, to guide them in finding their health and peace.

Sunghoon Kim FNP, is originally from South Korea and moved to the US in 2013. From South Korea, Kim received a BA in English Education. After moving to the US, he received his BSN from the University of Oklahoma, OK. Kim went on to work in the surgical trauma ICU in Kansas and completed his MSN FNP in Cedarville University, OH. Sunghoon Kim values the significance of preventive medicine and wants to contribute to keeping the community healthy and increasing the well-being of the community. He loves spending time with his family and hiking in the outdoors.

Nicole Goodwin FNP, is a board-certified Family Nurse Practitioner. She earned her master's degree from the University of Pennsylvania in December 2019 in Philadelphia, PA. She graduated with her undergraduate degree in nursing from Northern Arizona University in Flagstaff in 2012. Nicole worked as an Emergency Department RN in Phoenix, Arizona and then as a travel nurse in Maryland, New Hampshire, South Carolina, Pennsylvania, and California serving suburban, urban, rural, and critical access communities. As a Nurse Practitioner, she has been working in rehabilitation medicine and primary care at a skilled nursing facility where she has gained experience in post-operative management and geriatric medicine. Seeing the overuse of emergency departments all over the country has ignited a passion for Nicole to encourage and educate patients about holistic care and preventative medicine. She looks forward to forming meaningful relationships with her patients and allowing them a safe space to express their concerns and ask questions. In her free time, Nicole enjoys hiking, photography, and painting. Her primary passion is traveling the world and having adventures in new places. She is excited to be back in Flagstaff after her time away and join the Apricus team at Flagstaff Family Care.


Janine Cwiklinski MA LPC NCC

Janine Cwiklinski (pronounced “Quick-Lynn-Ski”) comes to us with over 20-years of behavioral health experience. She is originally from Philadelphia and has called northern Arizona her home now for over ten years. Her educational background is varied studying liberal arts, creative arts therapy, a variety of body psychotherapy modalities, and counseling psychology. Janine has worked with a variety of people across their life span and enjoys supporting others in finding solutions to their challenges. Creating wellness and restoring a sense of balance has been a professional passion that she looks forward to sharing with our patients.

Michele Peters FNP, graduated from University of Phoenix with a Master of Science degree in Nursing and Family Nurse Practitioner and undergraduate Magna Cum Laude with Bachelor of Science degree in Nursing from Grand Canyon University. She held positions as adjunct professor at Grand Canyon University in the Family Nurse Practitioner Program and Pharmacology. Michele’s nursing career includes Barrow Neurological Institute and Mayo Clinic. For the past 15 years she has been in independent practice as a family practice nurse practitioner in primary care, with a focus on adult and pediatric neurology. Additionally, she is certified in Vegas Nerve Stimulator and Deep Brain Stimulator adjustments and manages neurological conditions such as: Multiple sclerosis, Parkinson's Disease, Seizure Disorder, Dementia, Headaches, Neuropathy, Tremors etc.


Nick J. Paslidis, MD, PhD, MHCM, FACP, FABQAURP

Dr. Nick Paslidis has 32 years of Internal medicine and Gastroenterology clinical practice with progressive healthcare management experience as a physician executive in acute care hospitals and systems and health plans. He spent from 1996-2010 in various clinical and leadership positions in Arkansas including CEO of the Statewide Quality Organization and CMO in Northern Arizona Healthcare. Dr. Paslidis received his BA in Chemistry in 1981 and MD in 1988 from Ross University School of Medicine graduating as valedictorian. He went on to earn High Honors completing his PhD in Cell Biology from MD Anderson Cancer Center- University of Crete School of Medicine in Greece in 1991 and a Masters in Healthcare Administration from Harvard University in 2006. He received Post Doctoral research training at Baylor College of Medicine, then Internal Medicine internship and residency at University of Texas Medical School- Houston. Went on to training in Gastroenterology at Harvard School of Medicine- Brigham and Women’s Hospital- Dana Farber Cancer Center. Elected as a Fellow of the American College of Physician in 2018.
