Dr Chad Taylor - New Practitioner Announcement

May 27th, 2021
We are pleased to announce the addition of our new practitioner joining our Flagstaff 4th St office seeing dermatology patients Thursdays 8am-5pm! Dr. Chad Taylor was born in Fargo North Dakota, but was raised in Eden Prairie, a little suburb outside of Minneapolis. He attended Minnesota State University in Mankato where he earned his undergraduate degree in Human Biology with Minors in Chemistry and Political Science. After his undergraduate education, Dr. Taylor attended medical school at A.T. Still University where he received his Doctorate in Osteopathy. During his Family Medicine residency in Kingman, AZ, Dr. Taylor was recruited to join a dermatology team with the professionals at Mohave Skin and Cancer Clinic where he practiced and received special training for 5 years. Dr. Taylor believes in a cohesive, patient-centered, multidisciplinary clinical approach to offer the best medical care possible to his patients. On his free time, he enjoys running, playing racquetball, playing guitar, reading, writing, and spending time with his family.


Cameron Allen, PA-C - New Practitioner Announcement

We are pleased to announce the addition of our new practitioner joining our Flagstaff Yale office! Cameron Allen, PA-C, is a Phoenix native who graduated from Northern Arizona University with a Masters in Physician Assistant Studies. During his undergraduate studies he studied health sciences at Arizona State University while working as a pharmacy technician. Cameron has a passion for preventative medicine, research and public health initiatives. He is excited to join our Flagstaff team and better the health and wellness of the community. In his free time, Cameron is an artist and avid musician with a love for electric guitar, bass, and synth.

Dr. Keith Arnold - New Practitioner Announcement

We are pleased to announce the addition of our new practitioner! Dr. Keith Arnold is a board certified Family Medicine Physician. He recently completed his training at the Mountain Area Health Education Center in Asheville, North Carolina. He is a graduate of University of Arizona Medical School where his interest in community health, leadership and policy inspired him to complete the combined Masters of Public Health / M.D. program. As an undergraduate Keith attended U.C. Berkeley and studied International Political Economy. His passion for medicine arises from personal experiences caring for loved ones during his youth and integrates his desire to build meaningful relationships and uplift communities. Dr. Arnold treats patients across the lifespan from newborn to geriatrics. His areas of focus include: early childhood development, newborn, pediatrics, in-office procedures, preventative care, mental health and substance abuse treatment. He believes in partnering with patients to effectively manage chronic conditions and achieve mutual health goals. He looks forward to providing holistic, patient-centered care to the residents of Flagstaff and beyond. Se habla español.


Michele Peters, FNP - New Practitioner Announcement

We are pleased to announce the addition of our new practitioner! Michele Peters FNP, graduated from University of Phoenix with a Master of Science degree in Nursing and Family Nurse Practitioner and undergraduate Magna Cum Laude with Bachelor of Science degree in Nursing from Grand Canyon University. She held positions as adjunct professor at Grand Canyon University in the Family Nurse Practitioner Program and Pharmacology. Michele’s nursing career includes Barrow Neurological Institute and Mayo Clinic. For the past 15 years she has been in independent practice as a family practice nurse practitioner in primary care, with a focus on adult and pediatric neurology. Additionally, she is certified in Vegas Nerve Stimulator and Deep Brain Stimulator adjustments and manages neurological conditions such as: Multiple sclerosis, Parkinson's Disease, Seizure Disorder, Dementia, Headaches, Neuropathy, Tremors etc.


Candace Patel, FNP - New Practitioner Announcement

We are pleased to announce the addition of our new practitioner! Candace Patel is a Family Nurse Practitioner and one of our newest additions to Flag Family Care. She completed the Family Nurse Practitioner program at Georgetown University in Washington D.C in May 2019. She has previously worked as a Women's Health Nurse Practitioner with Gallup Indian Health Service before relocating to Flagstaff. She graduated from Arizona State University in December 2011 with her Bachelor of Science in Nursing. Her background as a Registered Nurse includes the Intensive Care Unit for 6 years and the Emergency Dept for 1 year. Candace was born and raised in Arizona, and recently moved back to Flagstaff to enjoy the mountainous landscape and opportunities this town has to offer. She has three young children and a wonderful husband. Candace enjoys being a primary care provider and is passionate about preventative care and health counseling. She looks forward to serving the beautiful community of Flagstaff, Arizona.


New Flagstaff Office Location

Beginning Monday March 16th, our Beaver office is moving to a new location at 2001 N Fourth Street, Flagstaff, AZ. 86004. We will no longer see patents at the previous building on Beaver St. The Yale office location will be open for business as usual.

Flagstaff Family Care Clinic introduces behavioral health services

Flagstaff Family Care Clinic added a new pillar to its practice in the way of a behavioral health provider. The new program will work in tandem with the clinic's primary care providers to provide the best care for our patients.

Janine Cwiklinski (pronounced “Quick-Lynn-Ski”) comes to us with over 20-years of behavioral health experience. She is originally from Philadelphia and has called northern Arizona her home now for over ten years. Her educational background is varied studying liberal arts, creative arts therapy, a variety of body psychotherapy modalities, and counseling psychology. Janine has worked with a variety of people across their life span and enjoys supporting others in finding solutions to their challenges. Creating wellness and restoring a sense of balance has been a professional passion that she looks forward to sharing with our patients.

Dr Mary Porvaznik - New Practitioner Announcement

We are pleased to announce the addition of our new practitioner! Mary Porvaznik M.D., will be seeing patients at one of our Flagstaff Office locations and our new Sedona Office location.

Dr. Mary Porvaznik is Board Certified in Family Medicine. She completed medical school at the Uniformed Services University of the Health Sciences in Bethesda, MD. She completed her Family Medicine Residency at the Medical University of South Carolina in Charleston, SC. She served in the US Public Health Service with the Indian Health Service for 24 years in various jobs and locations, including Tuba City, AZ and Shiprock, NM and a 4 year tour teaching back at her medical school in Bethesda, MD.
She moved to Flagstaff 2 years ago with her teenage daughters and husband to take advantage of the educational resources and opportunities the city has available. Dr. Porvaznik cares for people of all ages and all walks of life, to guide them in finding their health and peace.


Dr Rebecca Miller - New Practitioner Announcement

Effective Sept 3, 2018, We are pleased to announce the addition of our new practitioner! Rebecca Miller, M.D., will be seeing patients at one of our Flagstaff Office locations.

Dr. Rebecca Miller is a board certified family medicine physician and has completed a fellowship in Integrative Medicine at the Arizona Center for Integrative Medicine. She grew up in Tucson and graduated from medical school and residency at the University of Arizona. In addition to her traditional medical education, the fellowship in Integrative Medicine provided training in nutrition, supplements, and alternative techniques of healing. Rebecca enjoys seeing patients of all ages and backgrounds.

Karen Damgaard, PA-C - New Practitioner Announcement

Effective Dec 1, 2017, We are pleased to announce the addition of our new practitioner! Karen Damgaard, PA-C is a Flagstaff native. Born and raised in Flagstaff, she has always returned because she considers Flagstaff home. Karen earned a Bachelor of Fine Arts degree in Graphic Design from the University of the Pacific in Stockton, California. She later went to earn a Bachelor of Science degree in Zoology with a minor in Pre-med studies and Chemistry at Northern Arizona University. Prior to earning her Masters degree in Physician Assistant Studies at Idaho State University in Pocatello, Idaho, Karen worked for several years as a medical assistant in several private practices settings as well as a community health care facility. As a PA, Karen has spent the last 10 years serving the medical needs of the uninsured, underinsured, and privately insured people of Northern Arizona by continuing her work in a community health care center. She is excited to join the team at Flagstaff Family Care and looks forward to the challenges that lie ahead. Karen enjoys the field of family medicine because it allows her to practice medicine in a wide variety of areas while interacting with patients of all ages. Her greatest reward from practicing medicine is learning from her patients and watching them achieve optimal health.

In her spare time, Karen enjoys spending time outdoors and training for half-marathons, marathons and century bike rides. She also enjoys traveling, reading a good book and relaxing with her family, friends and dog.


Kathy Barth PA-C - New Practitioner Announcement

Kathy Barth, PA-C, graduated from Towson University, Towson, Maryland with a Masters in Science as a physician assistant. She also has an Education Master’s degree from SUNY at Buffalo and a Bachelor degree in Music from ASU. Kathy has worked as a teacher and managed continuing medical education departments for two medical specialty societies in the Baltimore/Washington, DC area. She has practiced medicine in rural Nebraska and at a community health center in Flagstaff. Kathy enjoys seeing all ages in family practice from 2 years old and up.


Elysia Small, PA-C - New Practitioner Announcement

Effective Oct 19, 2016, We are pleased to announce the addition of our new practitioner! Elysia Small, PA-C graduated from the University of Florida, Gainesville, FL in 2007 with a bachelor’s degree in Nutrition and a minor in Japanese Language and Literature. She has several years of experience as a Rehab Tech and CNA. She has volunteered in many capacities including as a nursing home activities volunteer and as a mentor through Big Brothers Big Sisters. She graduated in 2016 with a Master of Science in Physician Assistant Studies from A.T. Still University in Mesa, AZ and recently moved to Flagstaff, AZ to join the team at Flagstaff Family Care. She enjoys working with all ages including children.

During her free time, Elysia enjoys many outdoor activities including hiking, rock climbing, caving, and canyoneering. She also enjoys an occasional artistic endeavor and spending time with family and friends.
