Dr Mary Porvaznik - New Practitioner Announcement

We are pleased to announce the addition of our new practitioner! Mary Porvaznik M.D., will be seeing patients at one of our Flagstaff Office locations and our new Sedona Office location.

Dr. Mary Porvaznik is Board Certified in Family Medicine. She completed medical school at the Uniformed Services University of the Health Sciences in Bethesda, MD. She completed her Family Medicine Residency at the Medical University of South Carolina in Charleston, SC. She served in the US Public Health Service with the Indian Health Service for 24 years in various jobs and locations, including Tuba City, AZ and Shiprock, NM and a 4 year tour teaching back at her medical school in Bethesda, MD.
She moved to Flagstaff 2 years ago with her teenage daughters and husband to take advantage of the educational resources and opportunities the city has available. Dr. Porvaznik cares for people of all ages and all walks of life, to guide them in finding their health and peace.


Flagstaff Family Care Clinic introduces behavioral health services

Flagstaff Family Care Clinic added a new pillar to its practice in the way of a behavioral health provider. The new program will work in tandem with the clinic's primary care providers to provide the best care for our patients.

Sarah Bales is a Licensed Master Social Worker and the Behavioral Health Manager at Flagstaff Family Care. She graduated from Wesleyan College with a double major in History/Political Science and Women’s Studies. Prior to beginning graduate school, Sarah served in Americorps as a paralegal for Legal Aid. She also worked as a classroom interventionist, providing in-class behavioral and academic interventions to students and also taught sexuality education. Sarah earned her Master of Social Work with an emphasis in physical and behavioral health integration at Arizona State University. In the field of social work, Sarah has worked with homeless youth; hospice patients and their families; general mental health patients; and patients with serious mental illness. When she’s not providing care to patients, Sarah enjoys exploring the Southwest, hot springing, and wildcrafting local herbs. She has an affinity for learning new things, particularly in the area of bio-psycho-social wellness.


Dr Valerie Van Ravenswaay - New Practitioner Announcement

Effective Sept 3, 2018, We are pleased to announce the addition of our new practitioner! Valerie Van Ravenswaay, D.O., will be seeing patients at one of our Flagstaff Office locations.

Dr. Valerie Van Ravenswaay, grew up in Ontario, Canada where she practiced as a Registered Massage Therapist. She graduated from Ohio University Heritage College of Osteopathic Medicine where she completed an extra year in osteopathic medicine as a pre-doctoral fellow. She went on to complete her family medicine residency at Texas A&M Physicians in College Station. Her interests are in osteopathic medicine, preventive care, women's health, sports medicine. She is very excited to move to Flagstaff with her husband and two children to enjoy all of the outdoor activities that it has to offer.

In her free time, she loves to do marathon and triathlon and any outdoor activities while spending time with family and friends.


Dr Rebecca Miller - New Practitioner Announcement

Effective Sept 3, 2018, We are pleased to announce the addition of our new practitioner! Rebecca Miller, M.D., will be seeing patients at one of our Flagstaff Office locations.

Dr. Rebecca Miller is a board certified family medicine physician and has completed a fellowship in Integrative Medicine at the Arizona Center for Integrative Medicine. She grew up in Tucson and graduated from medical school and residency at the University of Arizona. In addition to her traditional medical education, the fellowship in Integrative Medicine provided training in nutrition, supplements, and alternative techniques of healing. Rebecca enjoys seeing patients of all ages and backgrounds.

Dr Donna Peace - New Practitioner Announcement

Effective Jan 11, 2018, We are pleased to announce the addition of our new practitioner! Donna Peace, M.D., will be seeing patients at our Beaver Office location.

Dr. Donna Peace is Board Certified in Family Medicine. She graduated from the University of Connecticut in 1984 with a degree in Biology and went on to do some research at Pfizer Pharmaceutical Company for a couple of years. Her involvement in the community exposed her to community support services for victims of violence and she decided to return to school and earned a Master’s in Social work from UConn. She worked in the social work field, in school health and hospital social work for a total of 10 years, including a couple of years in Africa as a Peace Corps volunteer, before returning to school to study medicine. She graduated from the State University of New York at Buffalo in 2000 and did a Family Medicine residency program through the residency consortium in Buffalo. She was awarded a National Health Service Corps Scholarship and came to Arizona in 2003 with her family to work on the Navajo Reservation. She eventually moved to the White Mountains of Arizona and had a solo private practice for 8 years before deciding to move to Flagstaff to allow her family to take advantage of the many wonderful education and cultural activities that Flagstaff has to offer. She provides care to all members of the family, from birth through childhood, adolescence and adulthood, into advanced years and even end of life care. She provides female care and does office procedures.

Dr. Peace enjoys caring for the entire family. She has four children and is involved in their interests and activities and is actively involved, in her own life, in applying the healthy living choices she prescribes for her patients.


Karen Damgaard, PA-C - New Practitioner Announcement

Effective Dec 1, 2017, We are pleased to announce the addition of our new practitioner! Karen Damgaard, PA-C is a Flagstaff native. Born and raised in Flagstaff, she has always returned because she considers Flagstaff home. Karen earned a Bachelor of Fine Arts degree in Graphic Design from the University of the Pacific in Stockton, California. She later went to earn a Bachelor of Science degree in Zoology with a minor in Pre-med studies and Chemistry at Northern Arizona University. Prior to earning her Masters degree in Physician Assistant Studies at Idaho State University in Pocatello, Idaho, Karen worked for several years as a medical assistant in several private practices settings as well as a community health care facility. As a PA, Karen has spent the last 10 years serving the medical needs of the uninsured, underinsured, and privately insured people of Northern Arizona by continuing her work in a community health care center. She is excited to join the team at Flagstaff Family Care and looks forward to the challenges that lie ahead. Karen enjoys the field of family medicine because it allows her to practice medicine in a wide variety of areas while interacting with patients of all ages. Her greatest reward from practicing medicine is learning from her patients and watching them achieve optimal health.

In her spare time, Karen enjoys spending time outdoors and training for half-marathons, marathons and century bike rides. She also enjoys traveling, reading a good book and relaxing with her family, friends and dog.


Patricia Bartholomew, FNP - New Practitioner Announcement

Effective Oct 2, 2017, We are pleased to announce the addition of our new practitioner! Patricia Bartholomew is a Family Nurse Practitioner and is a graduate of Northern Arizona University for her FNP. She began her career as a Marine Biologist as a graduate of Oregon State University. She worked on board a research vessel for several years before starting her path in a medical career. She started as a phlebotomist and a diener in pathology performing autopsies in San Health Sciences University in the development of the cryotherapy treatment for Retinopathy of Prematurity with Drs. Earl Palmer and Velma Dobson. From here she began her journey into a more fulling nursing career at the Providence Medical Center in the Pacific Northwest where she began her foundation in hospice and palliative care. Today she has found the perfect balance working as a nurse practitioner in Family Practice and in Hospice and Palliative care.


Kathy Barth PA-C - New Practitioner Announcement

Kathy Barth, PA-C, graduated from Towson University, Towson, Maryland with a Masters in Science as a physician assistant. She also has an Education Master’s degree from SUNY at Buffalo and a Bachelor degree in Music from ASU. Kathy has worked as a teacher and managed continuing medical education departments for two medical specialty societies in the Baltimore/Washington, DC area. She has practiced medicine in rural Nebraska and at a community health center in Flagstaff. Kathy enjoys seeing all ages in family practice from 2 years old and up.


Austin Grace PA-C - New Practitioner Announcement

Austin Grace, PA-C, graduated from the University of Notre Dame with a dual degree in English Literature and Political Science. He recently completed his Physician Assistant education at the University of New Mexico School of Medicine in Albuquerque. Prior to PA school he worked as an Orthopedic Technician at UNM Hospital and volunteered with Albuquerque Healthcare for the Homeless. He recently relocated to Flagstaff to join Flagstaff Family Care and is looking forward to serving the medical needs of the community while also enjoying all of the outdoor recreation activities in the area with his wife and their dogs.


Elysia Small, PA-C - New Practitioner Announcement

Effective Oct 19, 2016, We are pleased to announce the addition of our new practitioner! Elysia Small, PA-C graduated from the University of Florida, Gainesville, FL in 2007 with a bachelor’s degree in Nutrition and a minor in Japanese Language and Literature. She has several years of experience as a Rehab Tech and CNA. She has volunteered in many capacities including as a nursing home activities volunteer and as a mentor through Big Brothers Big Sisters. She graduated in 2016 with a Master of Science in Physician Assistant Studies from A.T. Still University in Mesa, AZ and recently moved to Flagstaff, AZ to join the team at Flagstaff Family Care. She enjoys working with all ages including children.

During her free time, Elysia enjoys many outdoor activities including hiking, rock climbing, caving, and canyoneering. She also enjoys an occasional artistic endeavor and spending time with family and friends.

Effective Oct 19, 2016, We are pleased to announce the addition of our new practitioner! Laurel Hatfield, FNP-C is our new board certified nurse practitioner. She is native to Arizona. She received her Bachelors in Nursing from the University of Arizona in May of 2003. She has specialized in medical, surgical, psychiatric and school nursing over the last thirteen years. Laurel earned her Masters of Science in Nursing with a Family Nurse Practitioner Certificate at Northern Arizona University in May of 2016. Laurel enjoys the management of acute and chronic conditions. She has a passion for integrative and holistic medicine. She looks forward to serving the community at Flagstaff Family Care.


Christine Crowder, M.D. - New Practitioner Announcement

Effective Aug 8, 2016, We are pleased to announce the addition of our new practitioner! Dr. Christine Crowder, M.D., is board certified in Family Medicine. She grew up in New York and graduated from Harvard University with the major of psychology and its biologic studies. She has done undergraduate research with the blind and supervised public service programs for student involvement in local communities in Boston. In medical school she enjoyed volunteering with the Philadelphia Boys and Girls Club and teaching disease prevention and health promotion for children. She graduated from the Medical College of Pennsylvania and Hahnemann University School of Medicine in Philadelphia. She then enjoyed working at Kaiser in San Francisco and relocated to Arizona to enjoy proximity to family. She appreciates her patients' trust and the caring of and learning from each one. Her greatest reward in medicine is watching her families grow through time.


Nitza Glick, PA-C - New Practitioner Announcement

Nitza Glick, PA-C, has been in the medical profession for over 25 years. She grew up outside of Boston and went to undergraduate school at The University of Massachusetts at Amherst. She then worked in Boston doing clinical research until relocating to Chicago Illinois to enter the Physician Assistant program at Cook County hospital. She has called Flagstaff home since 1997.

Dr. Cook is board certified in Family Medicine. She graduated with a major in Biochemistry from Rice University in Texas and received her medical degree from the University of Texas at Houston. She went on to family practice residency at John Peter Smith Hospital in Fort Worth, TX. She has traveled extensively overseas with her family serving in non profit work in Africa as well as India. She is excited to join the Flagstaff Family Care Clinic where she specializes in family medicine with an emphasis on pediatrics, women's health and urgent care needs. She and her husband are raising four children while serving the medical needs of the community of Flagstaff.
