Behavioral Health Service

Behavioral health examines how emotions, behaviors, and biology relate to a person’s mental and physical well-being; their ability to function in everyday life; and their concept of self. A person struggling with his or her behavioral health may face stress, depression, anxiety, relationship problems, grief, addiction, ADHD or learning disabilities, mood disorders, or other psychological concerns.

Behavioral Health Consultant can help with:

  • Coping with medical problems

  • Stress, Anxiety or Anger

  • Changing risky behaviors like tobacco and alcohol use

  • Difficult life problems or family problems

  • Child behavior problems

  • Eating and activity changes for improved health

  • Learning and memory problems

  • Assessments required for patients prescribed opioids

  • Learning and memory problems

  • And a variety of other mental health issues

Behavioral Health Service is now available at FFC. You can schedule a session with Flagstaff Family Care's very own behavioral health manager, Sarah Bales, LMSW. Appointments can often be on the same day as your medical appointment.

If you think that you or someone you care for may benefit from psychotherapy or another form of behavioral health treatment, the best place to start is with your primary care provider. Discuss your symptoms with them and ask them if it would be best for you to pursue care from a behavioral health clinician/manager.

What To Expect When I See The Behavioral Health Consultant

The Behavioral Health Consultant will meet with you and focus on the issues that are concerning you and your primary care provider. Together, you will work on a plan to improve the issue.

Often, patients only meet with the behavioral health manager once per month for a short duration to achieve their wellness goals.

Behavioral Health Consultant

Sarah Bales, LMSW

Sarah Bales is a Licensed Master Social Worker and the Behavioral Health Manager at Flagstaff Family Care. She graduated from Wesleyan College with a double major in History/Political Science and Women’s Studies. Prior to beginning graduate school, Sarah served in Americorps as a paralegal for Legal Aid. She also worked as a classroom interventionist, providing in-class behavioral and academic interventions to students and also taught sexuality education. Sarah earned her Master of Social Work with an emphasis in physical and behavioral health integration at Arizona State University. In the field of social work, Sarah has worked with homeless youth; hospice patients and their families; general mental health patients; and patients with serious mental illness. When she’s not providing care to patients, Sarah enjoys exploring the Southwest, hot springing, and wildcrafting local herbs. She has an affinity for learning new things, particularly in the area of bio-psycho-social wellness.
